If your child is not a US citizen and was born outside of the USA, you will need to complete the Proof of Entry Prior to 12 years of Age Waiver and provide a copy of an "official document" showing that he/she is living in the US now.  Examples of acceptable documents are copy of School Report Card, Copy of Medical Shot records.  If you completed this last year, you do not need to re-submit. Give to your Team Coordinator.

• Schedule of Fee Payments for all ages
• Additional Uniform Piece Order Forms (for items in addition to the standard uniform kit--give to Team Manager with payment). (Excel format)  (PDF format)
