Team Sponsorship Form for Warm-up Jersey Sponsorship

Medical Release (give to your Team Manager)              Notarized Medical Release (use only if required by your team manager)

Optional: 

• Financial Aid Application  (must be received by 6/1/11)  (Word format)  (PDF format)  
• Additional Uniform Piece Order Forms (for items in addition to the standard uniform kit--give to Team Manager with payment). (Excel format)  (PDF format)
• Schedule of Fee Payments for all ages

 Select Team Players Off-Field Code of Conduct  (this is a master form that is kept by the team managers.  The original is signed by all players and submitted to Compliance Officer--a copy is kept with the team manager).  Team managers--print this as a 2-sided document with code on the front side and the signature page on the reverse side.
