Select Team Players Off-Field Code of Conduct  (this is a master form that is kept by the team managers.  The original is signed by all players and submitted to Compliance Officer--a copy is kept with the team manager).  Team managers--only use the color printed 2-sided document with code on the front side and the signature page on the reverse side that was provided to you.  This is only required for U12 Elite and Palmetto teams in the Junior Academy as well as all Select teams.
